Instructions:-
- Filled Form-1 should be submitted to OC (Personnel) Control Room, O/o DC NRLM, Vikas Bhawan, Prayagraj .
- For filling Form-2 online on ERMS ULB22 software through Office Incharge login, mobile no. of the HOD (as mentioned in point 5.5 of Form-1) will be the user id and the OTP received thereon will be the password.
- Details of HOD and Nodal Officer should be filled at s.no. 1 and 2 respectively. Also, details of all employees should be filled in decreasing order of Grade Pay.
- After filling all the details of employees, office need to be freezed on ERMS ULB22 online and signed printout to be sent to OC (Personnel) Control Room, O/o DC NRLM, Vikas Bhawan, Prayagraj .
